AIDS Healthcare Foundation
The
AIDS Healthcare Foundation (AHF) has operated four healthcare
centers to provide direct medical services to individuals living
with HIV/AIDS under a capitated system of care.
Center for Community
Health, Education, and Research/Haitian Community AIDS Outreach Project
The
CCHER is a culturally
competent psychosocial educational counseling program that has
addressed the emotional and educational needs of HIV-positive
Haitians in Boston.
Cook County
Hospital/Hektoen Institute
The
project has ensured HIV education, counseling, and testing by
consent in family planning and perinatal sites in Cook County,
Illinois.
University of
Colorado Health Science Center
The
University of Colorado Health Sciences Center project has increased
the knowledge, ability, and willingness of rural healthcare
providers to deliver services to at-risk and HIV-positive patients.
Center for Women
Policy Studies
The Metro DC Collaborative has reduced
cultural, linguistic, and organizational barriers to care experienced by
women living with HIV/AIDS.
East Boston
Neighborhood Health Center
The
East Boston Neighborhood Health Center (EBNHC) has provided
coordinated, non-fragmented, comprehensive care to individuals with
HIV/AIDS under a capitated system of care.
Emory University
The Emory University project has provided
training and education to correctional healthcare providers on delivering
comprehensive and appropriate care to inmates living with HIV/AIDS.
Health Initiatives
for Youth
Health Initiatives for Youth (HIFY) has
improved the health, well-being, and quality of life of young people who
are living with, or affected by, HIV.
Johns Hopkins
University School of Medicine
The Johns Hopkins University project reduces
financial barriers to adequate care for HIV/AIDS patients without
comprising the quality of care.
Indiana Community
AIDS Action Network
The
Indiana Community AIDS Action Network (ICAAN) project helped reduce
discriminatory barriers to employment, care, housing, and other social
services faced by individuals living with HIV/AIDS.
Larkin Street Youth
Center
Larkin Street Youth Center (LSYC) has reduced
barriers to care and has assisted homeless youth in the San Francisco Bay
Area in accessing services.
University of
Mississippi Medical Center
The University of Mississippi project has
increased the number of primary care practitioners throughout Mississippi
who are willing and capable of giving high-quality primary care to persons
with HIV/AIDS.
Missouri Department
of Health
This
statewide program created, implemented, and evaluated the effectiveness of
a coordinated system of care through a collaborative effort of the
Departments of Health and Mental Health.
Michigan Protection
and Advocacy Service
The
Michigan Protection and Advocacy Service (MPAS) project trained Community
Advocates throughout the state on HIV/AIDS legal issues to improve service
delivery systems.
New York State
Department of Health
The New York State Department of Health AIDS
Institute has developed, implemented, and evaluated a managed care system
for persons with HIV.
Outreach, Inc.
Outreach, Inc. has offered a wide array of
comprehensive services to African American substance abusers with HIV/AIDS
residing in or around housing developments in Atlanta, Georgia.
PROTOTYPES
PROTOTYPES WomensLink has reduced barriers and
increased access to care for women living with HIV/AIDS through the
provision of a comprehensive, “seamless” continuum of care and
services.
University of Nevada
School of Medicine
The University of Nevada School of Medicine
project has prevented or slowed wasting syndrome experienced by
individuals living with HIV/AIDS.
State University of
New York Health Science Center at Brooklyn
The State University of New York (SUNY) at
Brooklyn project has increased counseling and voluntary testing of
pregnant women and access to perinatal AZT protocols.
University of Texas
Health Science Center at San Antonio
The University of Texas has helped positively
impact changes in the service delivery systems for families living with,
or affected by, HIV/AIDS.
University of
Washington Center for STDs and AIDS
The University of Washington project has
increased knowledge of HIV-related neuropsychiatric disorders, especially
delirium, through training educational videos, and study guides.
University of
Vermont and State Agricultural College
The University of Vermont and State
Agricultural College project has reduced barriers to care experienced by
individuals living with HIV in rural areas.
Visiting Nurse
Association Foundation
VNAF’s Transprofessional Model has used an
interdisciplinary, case management approach – a blend of curative and
palliative services – that has improved services for end-stage AIDS
patients.
Washington
University School of Medicine
The
Helena Hatch Special Care Center in the Washington
University School of Medicine is a specialized medical care
clinic that has provided coordinated, comprehensive care to women
with HIV/AIDS.
Well-Being Institute
The Well-Being Institute is a comprehensive
community-based nursing project reducing access barriers for substance
abusing, HIV-positive women who “fall between the cracks” of the
health delivery system in Detroit.

the service delivery experiences of 27 Cooperative Agreement Projects on Innovative Models
of HIV/AIDS Care. These projects and the Evaluation and Dissemination Center
which produced this Knowledge Base were funded by the Health
Resources and Services Administration (HRSA), HIV/AIDS Bureau (HAB) as Special Projects of National Significance (SPNS)
between 1994 and 1999.
