Evaluation Report Forms for the Hartford Geriatric Nursing Initiative

All John A. Hartford Foundation Geriatric Nursing Initiative grantees participate in a cross-cutting evaluation conducted by The Measurement Group, the independent evaluator for this Initiative. The general evaluation design was developed during the first year of the grant through the consensus process with grantees, which include the American Academy of Nursing, the American Association of Colleges of Nursing, the Hartford Institute at NYU, and the five Centers of Geriatric Nursing Excellence along with the staff of the Foundation. The major domains assessed for each project has been agreed upon and, in all cases, implemented into self-administer electronic evaluation report forms which are currently being used for data collection. Individual – but parallel – forms have been developed for all major program components.

  • Hartford Centers of Geriatric Nursing Excellence (HCGNEs): Five Schools of Nursing funded by the Trustees of the John A. Hartford Foundation from January 1, 2001 to December 31, 2005 designed to build academic geriatric nursing capacity by providing enriched and increased opportunities for training, research, clinical model development, and policy development for nurses who would develop into leaders in the next generation of geriatric nursing.

  • Hartford Pre-Doctoral Scholars and Hartford Post-Doctoral Fellows: 31 pre- and 20 post-doctoral scholars chosen through a national competition as part of the Hartford Foundation's initiative aimed at building academic geriatric nursing capacity and receive up to $100,000 each to support their studies and research in the field of geriatric nursing.

  • Nursing School Geriatric Investment Program (NSGIP): Five schools selected by the John A. Hartford Foundation as Hartford Centers of Geriatric Nursing Excellence and awarded five-year grants. Seven other Schools of Nursing were invited to apply for small, three-year grants to improve small, specific areas of their programs in gerontological nursing.

  • Coordinating Center at the American Academy of Nursing (AAN): Since 2000, an active partner of the Building Academic Geriatric Nursing Capacity (BAGNC) Initiative that provides a home for a Coordinating Center to facilitate the activities of the programs and grantees who were part of the BAGNC Initiative, foster communications among grantees and the Foundation, and provide oversight of various kinds for the different programs.

  • John A. Hartford Institute for Geriatric Nursing: The Hartford Institute identifies and develops best practices in nursing care of older adults and infuses these practices into the education of every nursing student and the work- environment of every practicing professional nurse. The main goal of the Institute is to shape the quality of health care older Americans receive by promoting the highest level of geriatric competence in all nurses by raising the standards of nursing care.

  • Enhancing Geriatric Nursing Education for Baccalaureate Nursing Program: In 2002 AACN funded 20 projects to work on issues of undergraduate education through baccalaureate programs. The program is a broad and long-term attempt to improve the quality of training in geriatric nursing in the American Schools of Nursing and to increase the number of nurses trained who are competent in geriatrics.

  • Enhancing Geriatric Nursing Education for Advanced Practice Nursing Program: In 2002 AACN funded 10 projects to work on issues of graduate education through master’s programs. The program is a broad and long-term attempt to improve the quality of training in geriatric nursing in the American Schools of Nursing and to increase the number of nurses trained who are competent in geriatrics.

  • Creating Careers in Geriatric Advance Practice Nursing Program: 23 schools selected by the American Association of Colleges of Nursing (AACN) and its Advisory Committee that strive to improve the quality of care provided to older adults by increasing the number of trained geriatric advanced practice nurses.

The versions of the Hartford Geriatric Nursing Initiative Evaluation Modules posted on this site are in Adobe PDF format. This means that the response fields are not active and all option may not appear. However, these copies of the evaluation modules do convey the domains of focus and level of questioning involved in the evaluation of the Initiative.
