Welcome to SPNS/Fax: An Electronic Report from HRSA/HAB's SPNS Cooperative Agreements. In each issue of SPNS/Fax, we will highlight findings from the HRSA Special Projects of National Significance Program Cooperative Agreements. The projects have been funded to develop innovative models of HIV/AIDS care. SPNS/Fax reports are distributed every two weeks by fax machine to all subscribers. All issues of SPNS/Fax are also available at this Web site. Due to slight differences in the media, issues distributed by fax machine may appear slightly different from those posted on this Web site, but the content is identical.
On October 1, 1994, the Special Projects of National Significance (SPNS) Program of the Health Resources and Services Administration (HRSA) funded an HIV Innovative Model of Care Initiative consisting of 27 cooperative agreements, HRSA, and an Evaluation and Dissemination Center (EDC). The cooperative agreement funding mechanism was selected to encourage these projects to identify common goals and objectives; to enable projects to share technical expertise; to produce comparable and measurable outcomes for innovative models of HIV care; and to establish a cross-cutting evaluation of all cooperative agreement projects that span all regions of the United States. This SPNS/Fax briefly summarizes the Cooperative Agreement Steering Committees second year report, "Progress During the Second Year: A Summary Report". This second year report augments the previous volume (i.e., the first annual report) by examining the progress of the Steering Committee after two years. The first annual report can be accessed through the Internet by logging onto www.TheMeasurementGroup.com/edc.htm.
The Cooperative Agreement second year progress report focuses on the general activities of the grantees as well as their specific outcomes. As the same group of grantees completes its second year of funding, a number of specific activities have been undertaken with some very specific outcomes. The 27 cooperative agreement projects have made significant achievements in the provision of sensitive and appropriate care and services for people living with HIV/AIDS in large metropolitan areas, inner cities, and small rural towns across the country. The following is a list of accomplishments that have been made in an effort to improve HIV systems of care targeting specific minority persons with HIV/AIDS.
Identification of the special needs of persons traditionally underserved in the health care system and ways to overcome barriers to services encountered by these groups.
Identification of strategies for expanding the capacity and responsiveness of local HIV/AIDS service systems.
Implementation of innovative training for health and other service providers.
Development of: 1) "storefront" community facilities for people living with HIV/AIDS; 2) special facilities or programs for persons with late-stage AIDS; 3) prison-based programs; 4) common methods of calculating costs of services and identifying barriers encountered in developing managed care services; and 5) methods for establishing comprehensive AIDS care clinics.
Assessment of medical needs at different stages of HIV disease and costs and/or benefits of various types of treatment programs.
People with HIV/AIDS have special medical and social services needs that require comprehensive care strategies. Just as other community-based, public health and private sector health and social services providers work to meet the needs of people living with HIV/AIDS, the 27 cooperative agreement projects of the SPNS Program strive to develop effective innovative models of HIV care.
