HIV Testing History Form: Final Draft of June 11, 1996


 
Main Purpose The purpose of this module is to have a standardized way of asking about the HIV testing history of the individual. The questions also ask about what happened after the individual was tested.
Description
  • Items for this form were developed for and used in the HRSA study of HIV-infected drug abusers. Also these items were adapted for use in the 11 Adolescent SPNS Projects.
  • Administered in 3-5 minutes.
Target Projects for Form
  • Any project documenting HIV testing history for individual clients.
Major Evaluation Function
  • Characterize history and experiences of project clients.
  • Use history and experiences to predict treatment compliance, outcomes, and retention in treatment protocols.
