The purpose of this module is to have a standardized way of asking
about the HIV testing history of the individual. The questions also ask about what
happened after the individual was tested. |

- Items for this form were developed for and used in the HRSA study of HIV-infected drug
abusers. Also these items were adapted for use in the 11 Adolescent SPNS Projects.
- Administered in 3-5 minutes.
