Main Purpose	The purpose of this module is to record focus group questions, outcomes, and participants.
Description	The form allows the user to code the actual or approximate number of individuals impacted by major demographic categories. These data are collected at the aggregate level. The form also codes the types of participants and the purpose of the activity. Additionally, major questions and conclusions derived from the focus group are coded.
Target Projects for Form	Any project that conducts focus groups.
Major Evaluation Function	To document the usage of focus groups for defining service needs, designing programs, educating clients, developing outreach strategies, etc.
