Main Purpose The purpose of this module is to code need for specific services, current utilization of services, barriers, and facilitators to receiving services.
Description
  • A broad history of recent service utilization is conducted through interview. For each of a large number of services, the client is asked if he or she needs the services, receives the services, certain special questions for that service, the perceived difficulty of getting the service, and the barriers to obtaining the service.
  • Module 4A collects needs expressed and services received by the client.
  • Module 4B collects client perceptions of barriers and facilitators to services.
Target Projects for Form
  • Projects wishing to do a comprehensive documentation of service needs and/or barriers.
Major Evaluation Function
  • Levels of barriers may change over the course of the project or for individual clients as a result of program participation.
