• Code current levels of self esteem.

Content Domain - Use major, multidimensional self esteem scale. (Rosenberg).

Operational Data Items - ID Letters/ID Numbers, Site, Sub-Provider, Date, Staff Code, Client Gender, Items 1 through 10.
